29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:      a.  On or about July 18, 2018, employees were permitted to conduct demolition operations and an Engineering Survey had not been performed to determine the conditions of the four story east wall and the possibility of unplanned collapse of the structure.      b.  On or about August 1, 2018, employees were permitted to conduct demolition operations and an Engineering Survey had not been performed to determine the conditions of the north wall and the possibility of unplanned collapse of the structure.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person was not made as the work progressed to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material:     a.) Administration Building; northeast corner; On or about July 18, 2018, the employer did not ensure a competent person inspected the structure, prior to employees drilling, setting 4 1/2" rods with epoxy and installing pedestal mounts for the CC-360 Hydraulic Wall Saw, to detect for hazards resulting from deteriorated walls and overhead stones.    b.) Administration Building; northeast corner; On or about July 19, 2018, the employer did not ensure a competent person inspected the structure, prior to employees dismantling and removing three pedestal mounts and track for the CC-360 Hydraulic Wall Saw, to detect for hazards resulting from deteriorated walls and overhead stones.    c.) Administration Building; northeast corner; On or about July 23, 2018, the employer did not ensure a competent person inspected the structure, prior to employees setting up the CC-360 Hydraulic Wall Saw and making vertical cuts, to detect for hazards resulting from deteriorated walls and overhead stones.    d.) Administration Building; southeast corner; On or about July 24, 2018, the employer did not ensure a competent person inspected the structure, prior to employees drilling, setting 4 1/2" rods with epoxy and installing pedestal mounts for the CC-360 Hydraulic Wall Saw, to detect for hazards resulting from deteriorated walls and overhead stones.    e.)  Administration Building; southeast corner; On or about July 25, 2018, the employer did not ensure a competent person inspected the structure, prior to setting up the CC-360 Hydraulic Wall Saw and making vertical cuts, to detect for hazards resulting from deteriorated walls and overhead stones.    f.)  Roof of north Cell House On or about August 1, 2018, the employer did not ensure a competent person inspected the structure, prior to employees drilling, setting 4 1/2" rods with epoxy and installing pedestal mounts for the CC-360 Hydraulic Wall Saw, to detect for hazards resulting from deteriorated walls and overhead stones.    g.)  Roof of north Cell House; On or about August 2, 2018, the employer did not ensure a competent person inspected the structure, prior to setting up the CC-360 Hydraulic Wall Saw and making the sloped 6/12 cuts, to detect for hazards resulting from deteriorated walls and overhead stones.
