At 5218 Silver Star Rd, Orlando FL, at the south side of the McDonald's construction site, employees were exposed to a 13 foot fall hazard while using a two frame tubular welded scaffold without railings, without an access ladder and without cross braces on the wall side, during decorative exterior stone installation, in that, a competent person did not supervise the scaffold erection to assure that it was conducted by trained employees, on or about 08/02/2018.  b. At 5218 Silver Star Rd, Orlando FL, at the east side of the McDonald's construction site, employees were exposed a 6 foot fall hazard while using 4 single frame tubular welded frame scaffold without an access ladder and without cross braces only on one side, during exterior insulation and stucco work, in that, a competent person did not supervise the scaffold erection to assure that it was conducted by trained employees, on or about 08/02/2018.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a. At 5218 Silver Star Rd, Orlando FL, at a McDonald's construction site, employees were using a two frame scaffold which did not have guardrails installed on the top frame, did not have an access ladder, and did not have cross braces on the wall side and were also using 4 single frame scaffold sections one frame high with cross braces only on the one side. Employees involved in scaffold use were not provided training and education in the following:  1. The nature of fall hazards and any falling object hazards.  2. The correct procedures for dealing with erecting, maintaining, and disassembling the fall protection system and falling object protection system being used.  3. The proper use of the scaffold, and the proper handling of material on the scaffold.  4. The maximum intended load and load carrying capacities of the scaffold.  5. Any other pertinent requirements of Subpart "L" (Scaffolds) under 29 CFR 1926.  On or about 08/02/2018.
