Skinner St., Wichita KS:  An employee engaged in residential re-roofing activities directly next to an employee applying asphalt shingles using a pneumatic nailer was exposed to stuck by hazards in that eye protection was not provided and used.

Skinner St., Wichita KS:  Employees engaged in residential re-roofing activities from a 6:12 pitch roof were exposed to fall hazards of approximately 10 feet from the eave of the residence to the ground below in that fall protection training was not provided.
