29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: On or about July 20, 2018, at 8021 Peters Rd, Plantation, FL 33324, employees were operating aerial lifts to apply stucco to the exterior sides of a newly constructed high-rise apartment building, during which the employees were sitting, climbing and standing on the mid-rails and top-rails of the guardrail system, therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. On or about July 20, 2018, at 8021 Peters Rd, Plantation, FL 33324, employees were operating aerial lifts to apply stucco to the exterior sides of a newly constructed high-rise apartment building, during which a body belt/PFAS was not worn and/or a lanyard was not attached to the boom or basket anchor point, therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(9): Lanyards and/or vertical lifelines did not have a minimum breaking strength of 5,000 pounds (22.2 kN): On or about July 20, 2018, at 8021 Peters Rd, Plantation, FL 33324, employees were using a PFAS while they were operating a two-point suspension scaffold, for the purpose of applying stucco to the exterior sides of a newly constructed high-rise apartment building, during which knots were placed by the employees in the vertical lifelines at each floor level, so at to enable the employees to connect their lanyard to the lifelines without using the required rope grab device, which could have resulted in the vertical lifelines not having a minimum breaking strength of 5,000 pounds (22.2 kN), and therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
