29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    (a)  On or about July 28, 2018, and at times prior, the controlling contractor, Cheyenne Mountain Roofing, LLC, did not ensure MA Roofing, protected their employees from fall hazards while conducting roofing activities on a roof in excess of six feet above the lower working surface without the benefit of a fall protection being in place, in that their accident prevention program did not include frequent site inspections to prevent persistent and reoccurring hazards.    Cheyenne Mountain Roofing, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1169578, Citation Number 1, Item Number 1, and was affirmed as a final order on September 8, 2016, with respect to a workplace located at 1865 Doherty Drive, Colorado Springs, CO.
