29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 2201 East South Boulevard, Montgomery, Alabama:  On or about August 7, 2018 and at times prior; the employer failed to ensure the extension ladder his employees were using to gain access to the roof had its rails extended three feet above the landing to provide safe access exposing his employees to a fall hazard of 13.3 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) At 2201 East South Boulevard, Montgomery, Alabama:  On or about August 7, 2018 and at times prior; the employer failed to ensure employees performing roofing activities on a low slope roof had fall protection in place exposing his employees to a fall hazard of 13.3 feet to the ground below.    THE DUSTIN RAGLAND DBA DUSTIN RAGLAND WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501 (B)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1256781 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 3, 2017, WITH RESPECT TO A WORKPLACE LOCATED AT LOT 69 BRECKENRIDGE DR., MONTGOMERY, ALABAMA.
