Electrical safety-related work practices, including de-energization/isolation and/or insulation were not ensured to prevent inadvertent bodily contact with energized parts.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The subject wire had a stripped copper-end that was not capped or otherwise guarded to prevent inadvertent bodily contact.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
