29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking-working surface: (a) On or about 8/7/2018 - at CA-4 Block #3 Outlet of Washer area, the midrail was installed at a height of approximately 29 inches above the walking-working surface and the top edge of the top rail was approximately 44 inches above the walking-working surface. The midrail height should have been at approximately 22 inches from the walking-working surface.
