29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  a) Exterior of the Building - Articulated Boom on Main St - 4th Floor: Employee engaged in framing activities on the 4th floor was exposed to an approximately 50 feet fall hazard while climbing the rails of an articulated boom, on or about 6/9/18.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan.    a) Interior Stairway - 1st Floor to 4th Floor: Employees engaged in framing activities on the 4th floor were exposed to tripping hazards by utilizing the four story unfiled metal stairway to access the working area, on or about 6/9/18.
