29 CFR 1926.502(d)(6): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged:  29 CFR 1926.502(d)(6)(i): directly to webbing, rope or wire rope;  a. On or about August 8, 2018, located at Sofidel's Building C in Circleville, Ohio, employees were installing overhead cable trays while their fall protection lanyard snaphooks were engaged into the landyard's strap risking disengagement of the snaphook, thereby exposing the employees to a fall hazard of approximately 15 feet.

29 CFR 1926.502(d)(23): Personal fall arrest systems shall not be attached to guardrail systems, nor shall they be attached to hoists except as specified in other subparts of this Part.  a. On or about August 8, 2018, located at Sofidel's Building C in Circleville, Ohio, an employee was installing overhead cable trays while standing outside of a JLG aerial lift (Model number E400AJP Narrow) basket. The employees six foot fall protection lanyard was connected to the aerial lift's guardrail exposing the employee to a fall hazard of approximately 15 feet.
