Energy control procedure.  29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A-D of this section:   On or about April 25, 2018, the employer's machine specific energy control procedure for the CNC Router was not adequate; in that the procedure did not specify the steps to shut down the equipment and did not include requirements for testing the equipment to determine and verify the energy control effectiveness.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about August 8, 2018, the employer had not completed a written certification of the personal protective equipment workplace hazard assessment to include the name or the person certifying the evaluation, the date for assessment and identifies the document as the certification.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  On or about August 8, 2018, the employer had not provided an employee training on hazardous chemicals in the workplace. The employee had potential exposure to wood dust from operation of a CNC machine and to acetone during application of adhesive associated with inlay installation. Hazards associated with these chemicals include irritation of the skin, eyes and respiratory system.
