29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems or personal fall arrest systems:   a. Site - On or about July 31, 2018, employees engaged in roofing activities were not utilizing fall protection and were exposed to falls of over 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Instance a. Site - On or about July 31, 2018, the Werner 24 foot extension ladder utilized by employees to access the roof area did not extend 3 feet above the upper landing surface.  Instance b. Site - On or about July 31, 2018, the 16 foot extension ladder utilized by employees to access the roof area did not extend 3 feet above the upper landing surface.  Instance c. Site - On or about July 31, 2018, the 30 foot extension ladder utilized by employees to access the upper roof area did not extend 3 feet above the upper landing surface.
