29 CFR 1926.501(b) (13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. Kelly Mill Subdivision, Lot #61, Dahlia Drive, Cumming, GA - Employees were exposed to fall hazard.  The employee was not utilizing a means of fall protection while working and walking the top plate installing roof trusses approximately 18 feet above ground level of the newly constructed two-story residential home, on or about 08/09/18.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   a.  Kelly Mill Subdivision, Lot #61, Dahlia Drive, Cumming, GA (Jobsite) - Employee were exposed to fall hazard.  An employee was using a 24ft. portable extension ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to gain access to the upper level working surface, on or about 08/09/18.
