29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall:    29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.     a. Located at 394 Saddlebred Circle in Marysville, Ohio: On or about August 8, 2018, employees were observed attached to a rope lifeline that allowed a free fall distance of approximately fourteen feet and did not prevent contact with first floor stud walls below. The employer did not ensure that the lifeline of the personal fall arrest system was rigged appropriately.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.      a. Located at 394 Saddlebred Circle in Marysville, Ohio: On or about August 8, 2018, employees were setting trusses and installing sheathing to the roof trusses without fall protection, thereby exposing employees to a fall hazard of approximately twenty one foot.      Donato Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1258200, citation number 1, item number 2 and was affirmed as a final order on 10/10/2017, with respect to a workplace located at 540 Stable Street, Marysville, Ohio.

Located at 394 Saddlebred Circle in Marysville, Ohio: On or about August 8, 2018, employees were installing sheathing to the roof trusses attached to a rope lifeline that was not properly anchored, thereby exposing employees to a fall hazard of approximately fourteen foot.      Donato Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.502(d)(15), which was contained in OSHA inspection number 1258200, citation number 1, item number 3 and was affirmed as a final order on 10/10/2017, with respect to a workplace located at 540 Stable Street, Marysville, Ohio.
