Untrained forklift operators can be a hazard to other employees working in the direct vicinity, such as not ensuring attachments/platforms are secured to the forks before operation.

A) Blackrock Masonry, LLC on or about August 09, 2018, at the address of 37500 Chester Road, Avon, OH: The employer did not ensure the scaffolding platform used to elevate employees via forklift, was designed and/or evaluated by a qualified person. The platform width exceeded 10 inches beyond the wheel base of the lift, increasing the probability of the forklift overturning. Employees performing masonry activities from the platform in this condition were exposed to falls of approximately 25 feet.

A) Blackrock Masonry, LLC on or about August 9, 2018 conducting work at 35000 Chester Road, Avon, OH: The employer did not ensure a fabricated scaffold platform was securely attached to the forks of the JLG 1055 forklift. Employees preforming masonry work from the platform in this condition were exposed to falls of 25 feet.
