On or about August 9, 2018, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about August 9, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof nine (9) feet six (6) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.

On or about August 9, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about August 9, 2018, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
