29 CFR 1926.451(b)(2):  Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway were not at least 18 inches (46 cm) wide:    a) Employees were standing on single wood planks (9 inches wide) when erecting the scaffold and did not use fall protection when hoisting up components of the scaffold.  Location: 125 Canal Street, New York, NY    On or about 8/1/2018    NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds:    a) The employer did not have employees prepared for use of fall protection when erecting scaffolds; an employee was not wearing a fall protection harness and was exposed to 14 ft fall hazard standing on a single plank without fall protection.
