29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person.    A) McConnell Excavating LLC on or about August 09, 2018, at the address of 37500 Chester Road, Avon, OH: The employer did not ensure the scaffolding platform used to elevate employees of a subcontractor via forklift, was designed and/or evaluated by a qualified person. The platform width exceeded 10 inches beyond the wheel base of the lift, increasing the probability of the forklift overturning. Employees performing masonry activities from the platform in this condition were exposed to falls of approximately 25 feet
