Scaffold frames were used as a means of access. Location: a) Supported scaffold on the ground level on the southeast side of the job site: On or about 08/06/18, an employee climbed the scaffold frames as a mean of access from the ground level to the first tier platform of a 2-tier-high supported scaffold.

The extension ladder is not extend at least 3 feet above the upper landing surface being accessed or not secured at its top to a rigid support or was not provided a grasping device.
