29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about 8/9/2018 at 415 Pine Grove Avenue Grayson, GA: where the employer failed to ensure that the workers observed performing roofing panel installation efforts were accessing the roof with a portable ladder that did not extend 3-feet above the access point exposing them to 14-feet fall hazards from the 6/12 pitch roof.
