29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 10, 2018, and at times prior thereto, employees were roofing the commercial project under construction approximately 17 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 10, 2018, and at times prior thereto, the employer did not provide a training program for all employees exposed to fall hazards. Note: The program shall include training and instruction in the following areas: a) The recognition and identification of fall hazards in the work area; b) The use and operation of guardrail systems (including perimeter safety cable systems), personal fall arrest systems, positioning device systems, fall restraint systems, safety net systems, and other protection to be used; c) The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; d) The procedures to be followed to prevent falls to lower levels and through or into holes and openings in walking/working surfaces and walls; and e) The fall protection requirements of this subpart.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access: On or about August 10, 2018, employees were performing roofing work approximately 17 feet high using a ladder to access the roof that did not extend at least 3 feet above the upper landing surface, exposing the employees to a fall hazard.
