All repairs shall be made by authorized personnel: On or about 08/01/2018, employee engaged in pallet building, operating Nissan Forklift, Serial #CPH01A15PV that had: a missing seat belt, inoperable horn, malfunctioning ignition switch, worn pedal pads, and had not been removed from service.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level: On or about 08/01/2018, exit located at the rear of the building was blocked by two parked vehicles, potentially exposing employees to hazards in the event of an emergency evacuation.
