29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces.  On or about August 08, 2018, and at times prior thereto, the employer did not effectively inform employees of the location and dangers of permit spaces.  There were no warning signs placed on or near a 48-ton sand storage hopper alerting employees that it is a permit-required confined space.

On or about August 08, 2018, the employer failed to take the necessary steps to prevent employee(s) from entering a confined space (48-ton sand storage hopper).
