29 CFR 1910.215(d)(1): Spindle speed(s) on grinding machine(s) were not checked before mounting to assure that it did not exceed the maximum operating speed marked on the wheel:      A.)  Prenatt Manufacturing, Inc.; Grinding Room - On or about 8/13/18, and at times prior thereto, swing free machine grinder/cut off wheels were rated for a lower RPM than then grinder.  An accident occurred on 7/24/18, when the grinding wheel exploded and causing an employee to sustain a laceration and broken right leg.  The grinder speed was 3450 RPM and the cutting wheel was 2260 RPM.

The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1, which is incorporated by reference as specified in Sec. 1910.6.  Section 3.3.1 of the CGA Pamphlet P-1 refers to storing cylinders in accordance with CGA and NFPA standards  The CGA Pamphlet P-1Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling."      A.)  Prenatt Manufacturing, Inc.; storage area - On or about 8/13/18, and at times prior thereto, compressed gas cylinders were not restrained.  The cylinders included, but were not limited to, argon, propane, and carbon dioxide.
