On or about August 13, 2018, jobsite located at 777 North State Road 7, Plantation, Florida 33317, the floor of the storage area above the service manager's office was not load rated for the intended storage of several rows of stacked tires and several rows of stacked boxes containing records, exposing employees to a struck-by and crushed-by hazard.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   On or about August 13, 2018, jobsite located at 777 North State Road 7, Plantation, Florida 33317, a pallet was mounted onto a forklift in order for an employee to gain access to the storage area above the service manager's office was being lifted and exposed to a fall hazard of approximately 9 feet 10 inches.
