29 CFR 1904.8(a): All work-related needlestick injuries and cuts from sharp objects that were contaminated with another person's blood or other potentially infectious material were not recorded on the OSHA 300 Log as an injury.  a) 9300 Dewitt Loop, Ft Belvoir, VA 22060: On or about August 15, 2018, and times prior thereto, sharps injury cases were not were not marked in column M(1) or other for cases that were classified on the form in Columns G, H, I, or J. The cases were not marked as an injury or an illness for 2013, 2014, 2015, 2016, 2017, and 2018 forms, and the error was carried forward to the 300A for each year.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent.  a) 9300 Dewitt Loop, Ft Belvoir, VA 22060: On or about August 15, 2018, and times prior thereto, the employer did not use an equivalent OSHA 300 or 300A in that the employer's forms did not have a Column M type for Hearing Loss cases, normally tracked in M(5). This data was not tracked on 2013, 2014, 2015, 2016, 2017, and 2018 forms, potentially exposing employees to injury that may have been prevented by accurate logs, tracking and trending injuries and illnesses, and training of employees.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours..  a) 9300 Dewitt Loop, Ft Belvoir, VA 22060: On August 11, 2018, at approximately 7:50 a.m., a registered nurse suffered an amputation of the right, middle fingertip. The employer did not report the amputation to the Occupational Safety & Health Administration until approximately 2:00 p.m. on August 13, 2018, after the 24-hour reporting period.  No abatement documentation or certification required.
