On or about August 15, 2018, a Kone 15 ton overhead ("North") crane was being used to move materials throughout the shop area.  The  employer did not ensure that the North crane was being inspected at frequent/regular intervals for defects that could present a safety hazard for employees working in the shop.  c.  On or about August 15, 2018, a Kone 15 ton overhead ("South") crane was being used to move materials throughout the shop area.  The employer did not ensure that the South crane was being inspected at frequent/regular intervals for defects that could present a safety hazard for employees working in the shop.

On or about August 8, 2018, at the work site located at 2195 Wright Brothers Avenue, Columbus, OH, employees were flipping a 14 foot long steel I-beam weighing 12,000 pounds so the other side(s) could be painted.  The flip caused the I-beam to suddenly swing horizontally and an employee was struck several times and pinned against the wall as a result of the sudden acceleration of the I-beam.
