Log entries from 2018, 2017, and 2016 did not record the classification of the injury/illness and/or properly detail the injury description (column f) on our about August 14, 2018.  a) On or about 5/1/18 an employee's foot was run over, log does not classify the kind of injury.   b) On or about 5/1/18 an employee's foot was run over, log does properly describe the injury as crushing.
