29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite: The employer did not ensure that employees were protected from falls while working on a scaffold at heights greater than 10 feet without the use of a fall protection system.  Condition was observed on or about 15 AUG 2018.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.  a) Jobsite: Employees were working from a scaffold without training or a competent person on site to ensure safety guidelines were followed.  Condition was observed on or about 15 AUG 2018.
