Crossbraces were used as a means of access.  a.  On or about August 15, 2018, at the above addressed jobsite, employees conducting masonry operations from a tubular welded from scaffold, approximately eighteen (18) above the ground, were not provided with an access ladder, thereby exposing the employee to falls.

On or about August 15, 2018, at the above addressed jobsite, an employee was conducting masonry operations was cutting masonry cinder blocks without engineering controls.  The employer did not perform an exposure assessment, thereby exposing employees to the hazards associated with respirable crystalline silica.
