29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about August 16, 2018, employees working in the shipping/dock area were exposed to fall hazards and the employer did not ensure fall protection was used: a. The #6 dock bay door was open and employees were exposed to approximately 49.5 inch fall hazard. b. The #8 dock bay door was open and employees were exposed to approximately 51 inch fall hazard. c. The #12 dock bay door was open and employees were exposed to approximately 50.5 inch fall hazard. d. The #14 dock bay door was open and employees were exposed to approximately 49.5 inch fall hazard. e. The #16 dock bay door was open and employees were exposed to approximately 50.5 inch fall hazard. f. The #18 dock bay door was open and employees were exposed to approximately 50.5 inch fall hazard. A trailer was docked at the time of inspection and the opening between the trailer and the bay opening was approximately 22 inches wide. g. The #20 dock bay door was open and employees were exposed to approximately 49.5 inch fall hazard. h. The #22 dock bay door was open and employees were exposed to approximately 49.5 inch fall hazard. i. The #21 dock bay door was open and employees were exposed to approximately 49 inch fall hazard. j. The #24 dock bay door was open and employees were exposed to approximately 50 inch fall hazard. A trailer was docked at the time of the inspection and the opening between the trailer and the bay opening was approximately 20 inches wide.
