A.) 765-Deck, LLC.; 2402 W. 15th Street, Erie, PA - On or about 8/16/18, and at times prior thereto, employees were engaged in roofing activities on a flat roof approximately 20 feet high. Employees were at the edge of the roof, outside of the warning line without fall protection, throwing old roofing material into a dumpster on the ground.

The company did not have a path created by rope or other substantial material from the ladder to access the roof to the warning line. B.) 765-Deck, LLC; 2402 W. 15th Street, Erie, PA - On or about 8/16/18, and at times prior thereto, employees were engaged in roofing activities on a flat roof approximately 20 feet high. The company did not have a path from the warning line to the area where material was being thrown off the roof into a dumpster. .

The company did not have a path created by rope or other substantial material from the ladder to access the roof to the warning line. A.) 765-Deck, LLC.; 2402 W. 15th Street, Erie, PA - On or about 8/16/18, and at times prior thereto, employees were utilizing a ladder to access the roof. The ladder was not extended at least 3 feet above the landing platform. The ladder was only extended 1 1/2 feet above the roof.
