29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a- 93 Wishing Well Lane, Lot 57 - Block 242.03, Sewell NJ: Company employees not wearing hard hats/head protection, were exposed to struck by hazards when working beneath company workers performing masonry activities using unsecured hand tools and equipment.  Observed on or about 08/18/18.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a- 93 Wishing Well Lane, Lot 57 - Block 242.03, Sewell NJ: On or about 08/18/18, a company employee was exposed to fall hazards in excess of 13 feet to the ground surface below when working and walking from a supported scaffold whose walking and working surface was not fully planked between the front uprights.       Guzzo Masonry Contractors, Inc./Guzzo Construction Managers was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.451(b)(1), which was contained in OSHA inspection number 1263567, citation number 2, item number 1 and was affirmed as a final order on 01/25/2018, with respect to a workplace located at 700 East Huntington Park Avenue, Philadelphia PA.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(e)(6)(vi):  Integral prefabricated scaffold access frames being used for access had a spacing greater than 16 3/4 inches.    a- 93 Wishing Well Lane, Lot 57 - Block 242.03, Sewell NJ:  On or about 08/18/18, company employees were exposed to fall hazards in excess of 13 feet to the ground surface below when accessing and egressing the working levels of supported and job built scaffolds using end frame openings that exceeded 16 3/4 inches.      Guzzo Masonry Contractors, Inc./Guzzo Construction Managers was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.451(e)(1), which was contained in OSHA inspection number 1110084, citation number 1, item number 3 and was affirmed as a final order on 07/21/2016, with respect to a workplace located at 6 Columbus Court, Berlin Township NJ.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a- 93 Wishing Well Lane, Lot 57 - Block 242.03, Sewell NJ: On or about 08/18/18, company employees were exposed to fall hazards in excess of 12 feet to the ground surface below when working from: a job built supported scaffold that was missing a mid and guard rails and a supported scaffold, along with its side brackets, that was not supplied with any guardrails and end stops.     Guzzo Masonry Contractors, Inc./Guzzo Construction Managers was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.451(g)(1), which was contained in OSHA inspection number 1110084, citation number 1, item number 5 and was affirmed as a final order on 07/21/2016, with respect to a workplace located at 6 Columbus Court, Berlin Township NJ.
