29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 1301 Goshen Dr., Lee's Summit, MO 64064:  Employees were exposed to a struck-by hazard.  Employees were exposed to injuries to the eyes and the face while using various pneumatic nail guns without eye protection.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  At a residential construction site located at 1301 Goshen Dr., in Lee's Summit, MO: Employees were exposed to struck-by and crushing hazards. The employer failed to protect employees in that the operation of a rough terrain fork truck with a basket attachment was observed without the approval for usage of the attachment by the manufacturer.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   At the residential construction site located at 4252 NE Tremont Ct, Lee's Summit, MO 64064:  Employees were exposed to fall hazards. Employees were performing framing operations at an elevated height of 20.4 feet without any means of fall protection. Employees were observed crawling along the trusses and roof decking without having their fall protection attached to an anchor point.
