29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: (a) Worksite: On or about 8/10/2018, an employee was exposed to a fall hazard of approximately seventeen feet from his fall protection hinged roof anchor not being fully secured to the peak of the roof.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) Worksite: On or about 8/10/2018, an employee was not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.
