29 CFR 1926.451(b)(1):   Each platform on all working levels of the scaffold was not fully planked or decked between the front uprights and the guardrail supports.  At a commercial construction worksite located at 103 N Evan Ave, Raymore, Missouri. Employee was working on a scaffold platform at an elevated height of approximately 12.5 to 14 feet, and the working surface was not fully planked or decked.

29 CFR 1926.451(g)(4)(i):   Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  At a commercial construction worksite located at 103 N Evan Ave, Raymore, Missouri. An employee was exposed to a fall hazard of approximately 12.5-14 feet, while working on a supported tubular framed scaffold platform without guardrails or any other means of fall protection.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial construction worksite located at 103 N Evan Ave, Raymore, Missouri. Employees were exposed to a fall hazard as they worked at the edge of a metal roof approximately 16.5- 18 feet from the ground below without using guardrails or any other means of fall protection.
