29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Pallet Facility: On August 9, 2018 and times prior, the employer did not ensure that machine guarding was provided on the drive of the notcher to protect employees from the rotating out feed belt during cleaning operations.

29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact.   (a) Pallet Facility: On August 9, 2018 and times prior, the employer did not ensure that following maintenance activities, machine guarding was reattached to the drive on the notcher to protect employees from the rotating out feed belt.
