29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Along the west side of the apartment building, on or about August 13, 2018, an employee performing siding work while standing on a pump jack scaffold platform more than 25 feet above the ground was not provided with adequate fall protection, resulting in a fall hazard.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary: a) Along the west side of the apartment building, on or about August 13, 2018, an employee was performing siding work while standing on a pump jack scaffold platform that had poles which were not secured to the bottom of the apartment building by rigid triangular bracing or equivalent, resulting in a fall hazard of more than 25 feet onto the ground.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: a) At the jobsite, on or about August 13, 2018, a stair rail system was not provided on the open side of the stairway that provided access from the first floor to the second floor, exposing employees to fall hazards of up to 11.2 feet onto concrete floor.
