29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a). At the worksite - On or about August 15, 2018, an employee used a pneumatic-powered nail gun without the use of eye protection, thus exposing the employee to struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a). At the worksite - On or about August 15, 2018, employees worked from the building's roof while conducting re-roofing activities without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 11.2 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a). At the worksite - On or about August 15, 2018, a portable extension ladder did not have its side rails extended at least three (3) feet above the roof's landing surface, thus exposing an employee to a fall hazard of approximately 8.7 feet.
