29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a). At the worksite - On or about August 15, 2018, employees worked from the building's roof while conducting re-roofing activities without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 11.2 feet.

At the worksite - On or about August 15, 2018, a portable extension ladder did not have its side rails extended at least three (3) feet above the roof's landing surface, thus exposing an employee to a fall hazard of approximately 8.7 feet.
