29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   (a) Not Your Average Joe's Roofing LLC at 4780 Vasquez Blvd Denver, CO 80216: On or before August 20, 2018 the employer did not insure that employees were protected from fall hazards while performing roofing work on a commercial building.  Employees were installing TPO at the edge of a flat roof approximately 18.5 feet above the ground without the use of fall protection.  Flagging barriers had been installed approximately 6 feet away from the edge, however employees were working past the barrier without the use of fall protection. This condition exposed employees to fall hazards of approximately 18.5 feet.
