29 CFR 1926.453(b)(2)(v): A body belt or harness, with lanyard attached to the boom or basket, was not worn by employee(s) working from an aerial lift:    a) North side of building, west end, on or about 8-16-18:  One employee was installing fascia and flashing while in an articulating, extensible boom, aerial lift, without a safety harness or body belt connected, exposing the employee to a fall hazard of 21 feet.

29 CFR 1926.454(c): The employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, and the employer did not retrain such employee so that the requisite proficiency was regained:    a) North side of building, west end, on or about 8-16-18:  One employee was working in an aerial lift while wearing a fall protection harness with leg straps that were not connected, exposing the employee to a fall hazard of 21 feet.
