29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about August 18, 2018, the employer did not ensure that employees working in areas where there was a danger of head injury from impact were protected by protective helmets. An employee of Josue Perez-Orellana was exposed to overhead hazards while disposing of debris on a dumpster at the same time that other workers were throwing debris from the roof above, employee was not wearing a helmet. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a) On or about August 18, 2018, the employer did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of Josue Perez-Orellana were exposed to fall hazards of up to 23 feet while engaged in roofing activities on a residential building with no fall protection systems in place. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about August 18, 2018, the employer did not ensure that side rails on portable ladders extend at least 3 feet above the upper landing surface being accessed. Employees of Josue Perez-Orellana were exposed to falls of up to 11 feet while descending a ladder that extended 1.5 feet above the landing surface.
