An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift.  On or about August 22, 2018, and at times prior thereto, employees were working in a trench preparing the installation of a moisture barrier for a three story apartment building up to approximately 8 feet deep without employee cave-in protection, such as but not limited to, hydraulic shoring, trench shielding, sloping or benching. Daily inspections were not conducted by a competent person.
