On or about August 22, 2018, and at times prior thereto, employees were working in a trench preparing the installation of a moisture barrier for a three story apartment building up to approximately 8 feet deep without employee cave-in protection, such as but not limited to, hydraulic shoring, trench shielding, sloping or benching. Daily inspections were not conducted by a competent person.
