29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. 885 Riverdale Avenue, West Springfield, MA: On or about August 23, 2018 employees on walking\working surfaces were not protect from tripping in or stepping into or through floor holes by guardrails or standard covers exposing them to falls at same height. (a) Employees working around elevator shaft on first floor with step into depth of approximately 55 inches. (b) Employees working around and near in ground pool with step into depth of approximately 60 inches. (c) Employees working around and near mechanical chase ways on second, third and fourth floors in stairwell wich were not covered to prevent step into hazards.
