On or about August 14, 2018, employees were operating the Chicago Dreis & Krump and Verson Mechanical Press Brake Machines and the foot pedals being used to actuate the machines were not guarded to eliminate employees from inadvertently actuating the foot pedal, thereby exposing employees to amputation hazards.
