29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employers: a) Job site, 3717 California Avenue, Pittsburgh, PA: On or about August 22, 2018, during painting and/or repair activities, the employer did not ensure that an appropriate system was in place to recognize and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, lack of fall protection being used, ladders not being properly extended above the landing level, failing to have a proper chemical safety program in place and allowing an extension cord to be used that had the outer jacket cut all the way through and covered with tape.
