29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4):  a) Dennis Group, LLC at the address of 216 Southgate Drive in Minster Ohio; On or about August 22, 2018, the employer did not ensure group lock out devices were affixed to pneumatic disconnects prior to performing service and maintenance activities in Danone North Americas Plant. IMS employees replacing the cluster valve body of valve 282AV1342 were exposed to hazards associated with high temperature liquids in that valve 282AV1230 was not isolated and locked out by an employee of Dennis Group. An employee suffered second and third degree burns to their arms, legs and mid-section when valve 282AV1230 was pulsed upstream and emptied into the drain been where the employee was working.
