29 CFR 1910.132(a): 29 CFR 1910.132(a): The appropriate protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:      a) 5546 Cochran Mill Rd.; On or about August 20, 2018 the employer exposed an employee to head injuries from falls in that the employee was not provided head protection while performing stunts from moving vehicles at speeds of approximately 18 mph.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  (a) Chattahoochee SR 70 - On or about August 20, 2018 and at times prior to, the employer did not certify that a hazard assessment was conducted for the hazards associated with the stunts being performed during the filming of the MacGyver television series.
