29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  5036 Hamilton Blvd., Allentown, PA, Worksite - on or about August 29, 2018, employees are performing roofing work on a steeply pitched commercial roof, at working heights of approximately 15 to 25 feet above grade, without utilizing any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  5036 Hamilton Blvd., Allentown, PA, Worksite - on or about August 29, 2018, employees are performing roofing work on a steeply pitched commercial roof while accessing the roof from a portable ladder that did not have its side rails extended at least three feet above the upper landing surface.
