Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance.

The flexible extension power cord, supplying power to portable fans in the smoker room, was damaged in proximity to the plug. The damaged cord was attached to the exterior of the swing doors leading into the smoker room, where employees could potentially make physical contact with the damaged plug, while entering the smoke room through the swing doors. The conductors of the live circuit were insulated and not exposed.  Effective support had not been provided to the flexible power cord to prevent damage. This condition exposed employees to an electrical hazard.
